Leave Forms

Q: How do I know what forms I need to complete for a leave of absence?

Employees that miss SIX (6) or more consecutive days due to illness are required to present a certificate from a physician certifying such illness.  This should be presented to your supervisor/principal.  The forms that need to be completed are determined by your reason for absence and leave available and/or leave balance. Please see the complete list below.

A) Reason for Leave  -->  Personal (Employee) Illness

Type of Leave --> Sick Leave
Complete the following Required Forms & Documentation:
Type of Leave --> Extended Sick Leave
Complete the following Required Forms & Documentation
Type of Leave --> Medical Sabbatical *(certified employee only)
Complete the following Required Forms & Documentation:
Type of Leave --> Leave Without Pay
Complete the following Required Forms & Documentation:
Employee should submit a letter to the superintendent for approval of any leave without pay of six (6) or more days.

B) Reason for Leave  -->  Illness of an Immediate Family Member Type of Leave --> Sick Leave

Type of Leave --> Leave Without Pay

C) Reason for Leave  -->  Death of an Immediate Family Member

Type of Leave --> Bereavement
 
Two (2) days are granted for the death of the following: (A spouse, child, mother, father, brother, sister, grandparent, grandparent of spouse, grandchild, Mother-in-law, Father-in-law, Brother-in-law, Sister-in-law, Daughter-in-law, or Son-in-law.)
 
Employee is Required to provide Verification to supervisor. (i.e. an obituary)
 
Type of Leave --> Sick Leave - (A husband, wife, children, mother, father, son-in-law, daughter-in-law, mother-in-law, father-in-law, brother, sister, grandparents, or grandchildren.)
 
An employee is allowed to use a maximum of three (3) sick days after using the two (2) bereavement days granted. Sick days are Required to be reported through the employee's standard Online Leave System.

Type of Leave --> Leave Without Pay
 
Required to provide the following Documentation:
 
If employee needs to take additional leave after using the two (2) bereavement days and three (3) sick days, employee should submit a letter to the superintendent for approval of leave without pay.
